If you would like to use insurance, please contact your insurance company prior to your first appointment to understand your eligibility and reimbursement benefits.
Here are a number of relevant questions to ask your provider to help you better understand your benefits. Have your insurance identification card ready and the name of your counselor. Call us for our federal tax identification number.
With this information, you may want to ask the following:
- Is this provider in my network? If yes, what is my co-payment amount?
- If not, do I have out-of-network benefits?
- What is my deductible?
- What percentage will the insurance company pay and what percentage do I have to pay?
- Where do I send the receipts for service?
For strictly phone counseling clients, please contact our office to set up an appointment with one of our staff. Download and print the client information forms and send them to the address below.
Our office hours are EST and are noted below. Phone counseling is not reimbursed by most insurance companies, therefore, for many of our clients a reduced fee is available.
Our reception office hours are from 9-5, Monday through Friday, except for major holidays in which case the office will be closed. For emergencies after hours, please contact any of the following:
